Age 70+ - 12 month “look-back”.   Exclusions (summary): 1) pre-existing conditions (unless conditions are met).
2) Mental/Nervous disorders unless hospitalized. 3) Pain Management /Treatment of chronic pain.
N.B. Congenital, genetic, chronic or degenerative diseases – see on right for pre-existing conditions / exclusions.
